Enrolling Grades K-12

The following forms may be downloaded in either Portable Document Format (PDF) or Rich Text File (RTF).  

The PDF files have the advantage that you can type in most of the information on this computer and submit the form directly to the school with the press of a button, as well as print a copy for your records.  You will need a copy of  Adobe Acrobat Reader to read them.

The RTF files have the advantage of being smaller.  They also can be read by virtually any word processor that you may have on your computer.  Unfortunately, you cannot fill them out online, but it is possible to add your information to them by editing them on your computer.  Just make sure that you do not remove any wording from the forms!

NOTE:  Some forms require signatures, so we must receive a signed copy from you before we can complete the application process.
